The Nurse Practitioner works as part of the clinic provider team performing a variety of direct patient care services including assessment, diagnosis, and implementation of treatment. The Nurse Practitioner effectively evaluates and monitors treatment care plans for clinic patients and adjusts care plan when goals are not met. The Nurse Practitioner performs these functions under a collaborative practice agreement with a licensed physician in a primary healthcare setting. Major Duties: Meets organizational expectations as defined by the Employee Standards of Performance, hospital and department policies and procedures, and the hospital's strategic goals. Verbalizes a clear understanding of continuous quality improvement and customer satisfaction measures and actively participates in improvement initiatives. Provides direct patient care in an ambulatory setting with a focus on wellness promotion and disease prevention. Obtains medical, social and family history from client, caregiver and/or previous medical records. Performs physical examinations on clients. Orders routine diagnostic testing as indicated, including but not limited to labs, x-rays, ultrasounds, etc., in collaboration with supervising physician. Interprets results of ordered diagnostic tests. In collaboration with clients, families and physicians, the Nurse Practitioner formulates, implements and monitors treatment plans for common acute and chronic illnesses. Consults with collaborating physician and other members of the healthcare team as necessary. Refers client to appropriate specialist(s) when indicated. Provides client and/or caregiver education and instructions regarding recommended treatments, therapeutic lifestyle changes, medication, disease processes, and prevention of disease. Documents all aspects of client care and accurately maintains medical record. Delegates responsibility to appropriate personnel, as well as, accepts delegated responsibility. Assists in providing staff education as deemed appropriate; serves as preceptor when requested. Maintains professional licensure as required by the Louisiana State Board of Nursing; effectively maintains standards of care and practices within the scope of an Advanced Registered Nurse Practitioner as outlined in the Nurse Practice Act. Verbalized understanding of continuous quality improvement, including meaningful use guidelines, as well as customer satisfaction measures; actively participates in improvement initiatives. Performs other duties as assigned or directed to ensure smooth operation of the organization and continuity of care for the patients. Education/Knowledge/Skills/Abilities: Preferred: Minimum one year of experience as a Nurse Practitioner in a primary care setting. Required: Advanced knowledge of nursing theory and nursing practice. Advanced knowledge of pathophysiology and pharmacology. Advanced assessment skills. Strong interpersonal skills. Ability to effectively educate patients and caregivers. Ability to maintain patient confidentiality at all times. Ability to communicate effectively with other members of healthcare team. Computer literate. Graduate of an NLN RN program and MSN nurse practitioner program. Licensure/Certification: Required: Current APRN licensure in Louisiana as a Nurse Practitioner. Current Louisiana RN license. Healthcare Provider CPR.
