This job assists with the medication reconciliation process by ensuring an accurate and complete home medication list is documented for patients admitted to Ochsner Medical Center. Communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain an accurate and complete medication history for patients; identify discrepancies or problems with medication history information and notify a pharmacist to seek clarification or resolution; ensure capture of appropriate patient information necessary to generate safe and effective medication orders (i.e. allergies, height, weight); ensure capture of prescription benefit information and document the home medication list in the electronic health record (EHR).Job Description

The Certified Pharmacy Medication Reconciliation technician provides technical support in obtaining, verifying accuracy and recording initial medication histories from patients and/or their designated family / caregiver. The Medication Reconciliation technician will have to conduct patient and/or family interview focused on obtaining a current medication history, which includes prescription medications, over the counter medications, vitamins and herbal supplements. Interview patients and /or designated caregivers as soon as possible after presenting to SMH for care in the Emergency Department. Obtain and record in the electronic medical record a current list of medications that includes prescription medications, over the counter medications, vitamins, and herbal supplements. Attempt to compile a complete medication history utilizing the patient, their caregiver and/or outpatient pharmacy records. Perform other functions as described in the SMH Certified Pharmacy Technician job description as needed. Successfully complete a medication history competency skills test. Understand metric and apothecary systems of weight and volume measurements. Know brand and generic names of top 200 drugs, different dosage forms available, pharmaceutical abbreviations.

Required: Louisiana license as a Certified Pharmacy Technician.

Required: High School graduate or equivalent

Preferred: Two years' Retail Pharmacy experience

The physical essential functions of this job include (but are not limited to) the following: Frequently exerting 10 to 20 pounds of force to move objects; occasionally exerting up to 100 pounds of force.
