Job Description

Occupational Therapist:
Provides direct occupational therapy services to patients. Demonstrates an appropriate level of clinical skill, as evidenced by the completion of comprehensive and insightful evaluations, and the development of individualized and effective treatment plans designed to achieve precise, functional goals. Evaluates patients in the areas of upper extremity function, activities of daily living, generalized physical functioning, psychosocial skills and cognition/perception in order to develop and implement treatment plans and goals. Uses excellent communication skills to educate patients and engage them in their own care. Stays updated regarding developments, new techniques, and equipment in the field of occupational therapy. Maintains a well-rounded knowledge base, and clinical reasoning skills to consistently achieve expected clinical outcomes.

Physical Therapist:
Provides direct physical therapy services to patients. Demonstrates an appropriate level of clinical skill, as evidenced by the completion of comprehensive and insightful evaluations, and the development of individualized and effective treatment plans designed to achieve precise, functional goals. Evaluates patient's physical and functional status and plans and implements therapeutic programs that include prevention, identification, correction and alleviation of acute or prolonged movement dysfunction of an anatomic or physiologic origin. Uses excellent communication skills to educate patients and engage them in their own care. Stays updated regarding developments, new techniques, and equipment in the field of physical therapy. Maintains a well-rounded knowledge base, and clinical reasoning skills to consistently achieve expected clinical outcomes.

Speech Therapist:
Provides direct speech therapy services to patients. Demonstrates an appropriate level of clinical skill, as evidenced by completion of comprehensive and insightful evaluations and the development of individualized and effective treatment plans, designed to achieve precise, functional goals. Evaluates patient's condition to diagnose and treat articulation, cognitive-communication fluency, language, resonance, swallowing and voice disorders. Responsible for assisting patients to develop and optimize their communication abilities and treat speech, language and voice disorders by administering prescribed speech-language therapy treatments. Uses excellent communication skills to educate patients and engage them in their own care. Stays updated regarding developments, new techniques, and equipment in the field of Speech-Language Pathology. Maintains a well-rounded knowledge base, and clinical reasoning skills to consistently achieve expected clinical outcomes.

In addition to clinical skill, demonstrates key practice management skills. Exhibits superior professionalism and people skills. Consistently maintains department productivity standards. Supports the activities of the entire team, including customer service, clinical outcomes and process improvement initiatives.

Qualifications

Occupational Therapist:
Required License and/or Certification with License:
-Current State of Illinois Occupational Therapist licensure
-Current American Heart Association BLS for healthcare provider or American Red Cross CRP/AED for the professional rescue certification
-A valid driver's license is required if the incumbent is selected to perform related duties at an off site location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.

Required without License:
-Authorization from the NBCOT to take examination
-Completion of exam within one year of date of submission
-Proof of scheduled exam date
-Letter of Authorization stating the applicant may practice work without their license under supervision
-Illinois Occupational Therapist license within 90 days of hire
-Current CPR Certification issued by American Heart Association BLS for healthcare provider or American Red Cross CPR/AED for the professional rescuer certification

Physical Therapist:
-Required License/Certification:
-Current State of Illinois Physical Therapist licensure
-Current CPR Certification issued by either American Heart Association BLS for healthcare provider or American Red Cross CPR/AED for the professional rescuer certification
-A valid driver's license is required if the incumbent is selected to perform related duties at an off site location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.

Required without License:
-Notification from Continental Testing Services advising of receipt and approval of application for licensure (if first-time test taker).
-"Authorization to Test" (ATT) letter from Federation of State Boards of Physical Therapy
-Scheduling and passage of exam within 60 days of receiving ATT letter and within eligibility listed on ATT letter
-Confirmation of Computer-Based Exam
-State of Illinois Physical Therapy license within 90 days of hire
-Current CPR Certification issued by American Heart Association BLS for healthcare provider or American Red Cross CPR/AED for the professional rescuer certification

Speech Therapist:
Required License/Certification:
-Current State of Illinois Speech-Language Pathologist license or a Temporary Speech-Language Pathology License issues by the Illinois Department of Financial and Professional Regulation
-Current CPR Certification issued by either American Heart Association or Red Cross
-A valid driver's license is required if the incumbent is selected to perform related duties at an off site location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.

Preferred License/Certification:
-If licensed, it is preferred to be American Speech-Language-Hearing Association (ASHA) certified (CCC-SLP)
